What to Expect in Aftercare

Your aftercare planning starts upon entering treatment. Our facility team collaborates closely with your case manager to develop a comprehensive aftercare plan tailored to your needs for when you’re ready to transition from our care.

Upon discharge, your case manager may continue to periodically check on you to ensure your well-being and make any necessary adjustments to the aftercare plan.

An ideal candidate for aftercare:

  • Demonstrates the desire and commitment to overcome addiction to drugs or alcohol

Even weeks, months, or years after completing a formal rehabilitation program, patients still face the risk of relapse. According to the National Institute on Drug Abuse (NIDA), the relapse rate for drug addiction is between 40-60%, with even higher rates for alcohol addiction. However, as time passes in sobriety post-rehab, the likelihood of relapse decreases.

At The Recovery Village, we prioritize post-rehab success by structuring our programs around the skills necessary for lifelong wellness. In the initial stages of treatment, patients acclimate to sobriety, learn to modify harmful thoughts and behaviors, and delve into the roots of their substance use disorder. After medical detox and residential treatment, they progress to increasingly independent levels of care, such as partial hospitalization, intensive outpatient, and outpatient programs. Here, individuals gradually gain autonomy, cultivate a robust support network, and develop deeper self-awareness and acceptance.

Upon completing these levels of care, individuals transition to the aftercare process. Aftercare signifies the shift from formal treatment to lifelong recovery beyond our facility’s confines. Before discharge, counselors, psychiatrists, psychologists, and physicians collaborate closely with each patient’s personal case manager to devise a comprehensive aftercare plan.
